2014 promises to be an exciting one for Westcott and our clients as we capitalise on the successes of 2013, expand across Hertfordshire and launch various new initiatives to help integrate care and support for the elderly across the county.
The last 12 months has seen us complete a successful pilot of our services. This has allowed us to improve the quality of our services and the way we interact with clients, care providers and other organisations involved in offering appropriate advice and support to our clients, e.g. GPs, hospitals, Hertfordshire County Council, financial and legal advisers.
We have grown our capacity and ability to meet the demand for our services, having completed the recruitment of a number of new Care Consultants: experienced local care professionals who are responsible and accountable for ensuring their clients receive the right care and support to meet their specific needs and wishes.
Although Westcott's core client base is elderly people who do not qualify for social services' support - either because they are ineligible for local authority funding or because their immediate needs are not critical enough to warrant intervention - we have been able to support Hertfordshire County Council in developing its use of independnet advisers and "brokers" to assist those receiving direct payments in finding the right care for them. As we look towards 2014, we plan to roll-out our services across the whole of the county and launch the Westcott Care Forum. This initiative is aimed at integrating the various elements of care and support for the elderly: healthcare, social care, well-being. The Westcott Care Forum brings together providers of care and support (such as care homes, home care agencies) with commissioners (such as the local authority, CCGs, GPs and, of course, individuals, families and carers). This new initiative will help to ensure that services are integrated appropriately, a named individual has a holistic overview, accountability and personal knowledge of an elderly person's requirements, and that clients reveive the right care at the right time to meet their individual needs and wishes.
